Knowing the wolf hell that is Milton, I'd take the distress pistol. Food is abundant in the town so it's not the most useful thing to take with you (In the end of Episode 1 I had more than ten kilograms with me) Meds are not going to be needed too much if you can avoid the wolves using the Distress Pistol, and if you need any there are plenty of natural medicine around.
