Dmitri, the diabetic, is the one that everyone's tripping on. Here's the thing: when you come back with Gwen and the insulin, we all instinctively go to each of the survivors once. But with Dmitri, you need to administer the insulin AND also give him the news about his loved one. 

As mentioned by others: talk to ALL the survivors and make it a case to also talk to Dmitri after giving him his shot.
