Regarding that "guaranteed to spawn" in the Ice Cave... Found a cache in the Ice Cave with the water falls (just before the Reclusive Falls Exit) but no Combat Pants :(. Never found these rare pants in Hushed River, three playthroughs now, most recent is Winter's Embrace other been custom runs with loot set to medium (just because low/interloper won't some things spawn). My experience is if you don't find them at Signal Hill in PV then there's a slight chance at the Milton House and if you scaled Timberwolf and still nothing you're not going to find them at all. But as with most high tier gear if you find one then you'll find more & more... looking at you Fisherman's Sweater... 5x and counting. TM, BR,FM,BI left.
