Outer layer is always the slot furthest away from the body on the "clothing" screen.  and False, not just cosmetic, besides the afore mentioned window protection difference, it also matters for waterproof % since the outer layer get wetter than the inner layer, you can test this by wearing two ski jackets and stand in the snow for a while.
